KONTOS, HERMES A., DAVID W. RICHARDSON, AND JOHN L. PATTERSON, JR. Roles of hypercapniu and acidosis in the vasodilutor response to hyperc@c acidosis. Am. J. Physiol. 2 15 (6) : 1406-1408. 1968.-The importance of increased Pcoz and decreased pH in the mediation of the effects of hypercapnic acidosis on heart rate, blood pressure, and blood flow and vascular resistance of the human forearm were investigated in normal volunteers. The responses to hypercapnic acidosis produced by 7a/, CO2 breathing were compared to the effects of hypercapnia without acidosis produced by 7Y0 CO2 breathing while at the same time isotonic NaHCOa solution was infused intravenously to maintain arterial blood pH at the resting level, Hypercapnic acidosis (CO 2 breathing without NaHC03 infusion) was associated with increased heart rate, increased mean arterial blood pressure, no change in vascular resistance of the intact forearm, and decrease in vascular resistance of the forearm previously treated with phenoxybenzamine and propranolol. These responses were not significantly different from those seen during hypercapnia without acidosis (CO:! breathing plus NaHCOa infusion). The results indicate that the effects of hypercapnic acidosis on heart rate, blood pressure, and on forearm blood vessels are mediated by increased blood (and tissue) Pcoz rather than by the associated decrease in pH.

CARDIOVASCULAR RESPONSES TO HYPERCAPNIA AND VARYING LEVELS OF ARTERIAL pH IN THE ANESTHETIZED CAT

Effects of acute hypercapnia on the cardiovascular system (CYS) were studied in the anesthetized cat. After surgery the animal was exposed to a gas mixture of 12% CO2 and 25% O2 in nitrogen, and hypercapnia with low levels of arterial pH (pHa) was produced for 20 minutes. In the second run the same level of hypercapnia was induced by ventilating the same cat from the above gas mixture but ...

Effects of hypercapnia with and without acidosis on hypoxic pulmonary vasoconstriction.

Acute respiratory disorders and permissive hypercapnic strategy may lead to alveolar hypoxia and hypercapnic acidosis. However, the effects of hypercapnia with or without acidosis on hypoxic pulmonary vasoconstriction (HPV) and oxygen diffusion capacity of the lung are controversial. We investigated the effects of hypercapnic acidosis and hypercapnia with normal pH (pH corrected with sodium bic...

Carbon dioxide in the critically ill: too much or too little of a good thing?

Hypercapnia and hypocapnia commonly complicate conditions that are present in critically ill patients. Both conditions have important physiologic effects that may impact the clinical management of these patients. For instance, hypercapnia results in bronchodilation and enhanced hypoxic vasoconstriction, leading to improved ventilation/perfusion matching. Hypocapnia reduces cerebral blood volume...

Moderate and prolonged hypercapnic acidosis may protect against ventilator-induced diaphragmatic dysfunction in healthy piglet: an in vivo study

INTRODUCTION Protective ventilation by using limited airway pressures and ventilation may result in moderate and prolonged hypercapnic acidosis, as often observed in critically ill patients. Because allowing moderate and prolonged hypercapnia may be considered protective measure for the lungs, we hypothesized that moderate and prolonged hypercapnic acidosis may protect the diaphragm against ven...

The Effects of Lung Protective Ventilation or Hypercapnic Acidosis on Gas Exchange and Lung Injury in Surfactant Deficient Rabbits

BACKGROUND Permissive hypercapnia has been shown to reduce lung injury in subjects with surfactant deficiency. Experimental studies suggest that hypercapnic acidosis by itself rather than decreased tidal volume may be a key protective factor. OBJECTIVES To study the differential effects of a lung protective ventilatory strategy or hypercapnic acidosis on gas exchange, hemodynamics and lung in...
